Acceptance of These Terms

Welcome to the website of Innovation Hydration Wellness Center ("Innovation Hydration," "we," "us," or "our").

By accessing or using our website, online scheduling services, patient resources, telehealth services, or other online features, you agree to these Terms of Use.

If you do not agree with these Terms of Use, please do not use this website or its online services.

Medical Disclaimer

Information provided on this website is intended for general informational and educational purposes and is not a substitute for individualized medical advice, diagnosis, or treatment.

Browsing our website, completing an online questionnaire, sending a message, or requesting an appointment does not by itself establish a healthcare provider-patient relationship.

A provider-patient relationship is established only when an appropriately licensed healthcare provider accepts you for care and provides services in accordance with applicable law and professional standards.

Do not disregard professional medical advice or delay seeking medical care because of information found on this website.

Medical Emergencies

Innovation Hydration's website and telehealth services are not intended for medical emergencies.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

Do not use our website, online scheduling system, email, social media accounts, text messaging, or telehealth scheduling system to request emergency medical assistance.

Telehealth Services

Innovation Hydration may offer healthcare services through telehealth.

Telehealth may allow patients to communicate remotely with an appropriately licensed healthcare provider through secure video, audio, electronic communication, or other approved technology.

Not every medical condition can be appropriately evaluated or treated through telehealth.

A provider may determine that laboratory testing, diagnostic testing, an in-person physical examination, urgent care, emergency care, specialist evaluation, or another form of healthcare is necessary.

Scheduling a telehealth appointment does not guarantee that a diagnosis can be made, medication will be prescribed, or a particular treatment will be provided.

Clinical decisions remain at the discretion of the treating healthcare provider.

Eligibility and Location

Patients using telehealth services may be required to confirm their identity and physical location at the time of the appointment.

Telehealth services are available only when the treating provider is legally authorized to provide care to a patient in the jurisdiction where the patient is physically located.

Innovation Hydration may decline or discontinue a telehealth encounter if legal, clinical, technological, or safety requirements cannot be satisfied.

Online Symptom Screening

Innovation Hydration may provide online questionnaires or screening tools to help patients select an appropriate appointment type.

These tools are intended to assist with appointment routing and information collection only.

They do not provide a medical diagnosis, determine whether a medical emergency exists, guarantee that telehealth treatment is appropriate, or replace the professional judgment of a licensed healthcare provider.

Based upon information provided by the patient, Innovation Hydration may recommend a Virtual Sick Visit, Express Care Visit, in-person evaluation, laboratory or diagnostic testing, urgent care, emergency care, or another appropriate level of care.

Prescriptions

When legally and clinically appropriate, a licensed healthcare provider may prescribe medication following an appropriate evaluation.

Scheduling or paying for an appointment does not guarantee that medication will be prescribed.

All prescribing decisions are made by the treating provider based upon the patient's medical condition, medical history, evaluation, applicable law, professional standards, and the provider's clinical judgment.

Innovation Hydration does not guarantee prescriptions for antibiotics, controlled substances, or any particular medication.

Testing and Express Care

Certain conditions may require an in-person component before or in connection with a telehealth appointment.

Innovation Hydration may offer an Express Care Visit that can include authorized clinical intake, vital signs, specimen collection, and/or appropriate diagnostic testing followed by evaluation by a healthcare provider.

Available testing and services may vary.

Testing does not guarantee a particular diagnosis or treatment.

Accuracy of Patient Information

Patients are responsible for providing complete and accurate information concerning their identity, symptoms, medications, allergies, medical history, pregnancy status when relevant, and other information reasonably necessary for their care.

Failure to provide complete or accurate information may affect the safety or effectiveness of treatment.

Appointments, Payments, Cancellations and Refunds

Innovation Hydration may require payment or a deposit when an appointment is scheduled.

Prices may vary depending upon the service provided, testing performed, medications administered, supplies used, or additional treatment requested or recommended.

Any applicable cancellation, late-arrival, no-show, or refund policy will be communicated during scheduling or displayed through our booking system.

Electronic Communications

By providing an email address or telephone number, you authorize Innovation Hydration to communicate with you regarding appointments, patient services, billing, and other practice-related matters as permitted by law.

Electronic communication carries inherent privacy risks.

Sensitive medical information should be submitted through communication methods designated by Innovation Hydration for healthcare information rather than through public social-media messages or unsecured website communications.

Separate consent may be obtained where required for promotional or marketing communications.

Privacy

Your use of our website is also subject to our Privacy Policy.

Medical information created or maintained in connection with healthcare services may additionally be governed by our Notice of Privacy Practices and applicable federal and state privacy laws.

Intellectual Property

Unless otherwise stated, the Innovation Hydration name, branding, website content, graphics, text, photographs, videos, educational materials and other original content are owned by or licensed to Innovation Hydration.

Content may not be copied, reproduced, distributed or commercially used without authorization except as permitted by law.

Third-Party Services

Innovation Hydration may use third-party services for functions such as electronic medical records, patient portals, scheduling, payment processing, telehealth, laboratory services, communications, analytics, or other operational functions.

Use of certain third-party services may also be subject to their respective terms and privacy practices.

Website Availability

Innovation Hydration attempts to maintain accurate and accessible online services but does not guarantee that the website, booking platform, patient portal, or telehealth system will always operate without interruption or error.

Technical difficulties during a telehealth appointment may require changing communication methods, rescheduling the encounter, or arranging an in-person evaluation.

Changes to These Terms

Innovation Hydration may modify these Terms of Use when our services, technology, policies, or applicable requirements change.

The current version will be posted on this website with an updated revision date.
